Aktuální stav projektu Albertinum Žamberk - září 2015

Pojďme si blíže představit projekt nazvaný „Albertinum Žamberk – zavedení systému ucelené rehabilitace v podmínkách psychiatrického oddělení“ a zejména si trochu přiblížit, co vlastně tento název skrývá a jaká je historie a současnost tohoto projektu.

První informaci o možnosti čerpat prostředky z grantu Norských fondů – Programu CZ 11 – „Iniciativy v oblasti veřejného zdraví“ – AKTIVITA I. Psychiatrická péče jsme obdrželi počátkem července loňského roku od vedoucího odboru zdravotnictví Pardubického kraje Ing. Gregora. A opět se ukázalo, že je dobré být připraveni, neboť jsme již pracovali na přípravě projektu rekonstrukce tzv. lékařské vily pro potřeby psychiatrického oddělení. Po zhodnocení našich kapacitních a časových možností jsme se rozhodli podat žádost o přidělení prostředků z grantu, vytvořili organizační a projektový tým a přes mimořádně „šibeniční“ termín (žádost měla být nejprve podána do 29.8.2014, tento termín byl později prodloužen do 12.9.2014) pro podání žádosti jsme zahájili spolupráci s Regionální rozvojovou agenturou Pardubického kraje (RRA Pk) na administraci této žádosti. A 12.9.2014, byť se to na počátku jevilo jako nemožné, jsme žádost podali a očekávali výsledek hodnocení, které probíhalo na MZ ČR jako partnera projektu a na MF ČR jako zprostředkovatele programu.

Tady je nutné poděkovat zejména paní Ing. Veronice Šimkové z RRA Pk za administraci a celkové zpracování žádosti, Ing. Tmejové a Ing. Věnečkové z KIP Litomyšl s.r.o. za nadstandardně rychlé zpracování projektové dokumentace pro stavební povolení a členům projektového týmu Mgr. Petře Urbanové, Mgr. Marcele Tomanové, Bc. Haně Leichtové a Ing. Vladimíru Fabiánovi za zpracování tzv. měkké části projektu a připomínkování stavební části projektu a vybavení. Dále je nutné poděkovat členům Rady a Zastupitelstva Pk a pracovníkům odboru zdravotnictví Pk za podporu realizace tohoto projektu a schválení investiční dotace ve výši 4,25 mil. Kč na spolufinancování.

Zpráva o tom, že námi podaná žádost o grant byla schválena, nám byla doručena z Ministerstva financí ČR dne 15. ledna 2015. Zároveň nám v Rozhodnutí o poskytnutí dotace byly potvrzeny celkové předpokládané výdaje na realizaci projektu ve výši 23 979 885,00 Kč, z čehož dotace z Národního fondu činí 19 183 907,00 Kč, což je max. 80% z celkových předpokládaných výdajů projektu. Náš podíl, jako investora a příjemce dotace činí 20%, tedy 4 795 978,00 Kč. Tuto částku budeme částečně hradit z investičního příspěvku zřizovatele Pardubického kraje ve výši 4 250 000,00 Kč, částečně uhradíme my vč. tzv. neuznatelných nákladů, které nemohou být hrazeny z grantu.

Ihned po obdržení informace o poskytnutí dotace jsme informovali zástupce zřizovatele a zahájili realizaci jednotlivých výběrových řízení na dodávky služeb. Zároveň jsme stanovili termíny schůzek projektového a organizačního týmu.

V pěti dosud ukončených VŘ byl vybrán administrátor projektu a dvou veřejných zakázek, kterým se stala Regionální rozvojová agentura Pardubického kraje (RRA Pk) a pověřenou administrátorkou je nyní Mgr. Petra Srdínková. Dále byla vybrána projekční kancelář, která v rámci předmětu díla zpracovala projektovou dokumentaci pro realizaci stavby a zajistí výkon funkce autorského dozoru při realizaci stavby. Tou je společnost KIP Litomyšl s.r.o. Ve výběrovém řízení na výkon technického dozoru investora stavební části projektu uspěla společnost Hudeček s.r.o. Ústí nad Orlicí. Zcela zásadním byl výběr zhotovitele stavební části projektu. Toto VŘ administrativně zajišťovala na základě smluvního vztahu RRA Pk a v soutěži uspěla společnost Chládek a Tintěra, Pardubice a.s. Prozatím posledním ukončeným VŘ je výběr koordinátora BOZP investora stavební části projektu. Tuto zákonnou povinnost zajistí smluvně Ing. Jan Dušek z Přelouče.

První průběžná monitorovací zpráva, hodnotící prozatímní průběh projektu za období od 14.01.2015 do 30.06.2015, včetně žádosti o platbu za fakturaci zhotovení projektové dokumentace pro realizaci stavby stavební části projektu byla MF ČR předložena k revizi a kontrole 31.07.2015. Nyní čekáme na případné připomínky či žádost o doplnění.

Dalším nezbytným krokem pro zajištění průběžného financování projektu je schválení úvěrových podmínek Radou a posléze Zastupitelstvem Pardubického kraje. Na tato jednání jsme předložili výsledek interního VŘ s doporučením výběru vhodného poskytovatele úvěru a věříme, že po jednání ZPk dne 17.9.2015 budeme moci uzavřít úvěrovou smlouvu a vše financovat.               

Hlavním cílem projektu „Albertinum Žamberk - zavedení systému ucelené rehabilitace v podmínkách psychiatrického oddělení“ je uspokojení podmínek pro rozvoj a implementaci komplexního programu léčebně rehabilitační péče v podmínkách psychiatrického oddělení, který umožní sledovat a vyhodnocovat stav pacienta a rozhodnutí o jeho umístění mimo léčebnou instituci do zařízení komunitní péče a postupně pacienta připravit na návrat do běžného života, což povede i ke snížení rehospitalizace pacientů. Komplexnost, resp. ucelenost systému je v předkládaném projektu chápána jako vzájemné propojení intervencí v oblasti tělesného zdraví, duševního zdraví a sociálních potřeb. Předmětem projektu je rekonstrukce stávajícího objektu tzv. "lékařské vily" v areálu našeho ZZ, neboť není možné implementovat kompletní systém ucelené rehabilitace do stávajících prostor psychiatrického oddělení. Rekonstrukcí vzniknou terapeutické místnosti k využití prvků skupinové psychoterapie a metod léčebné rehabilitace. Dále cvičná kuchyňka a denní místnost pro nácvik stravovacích návyků, stolování i k posilování fungování v běžném životě. Cvičná prádelna a doplňkové sociální zařízení k nácviku péče o osobní věci a zajištění sebeobsluhy nezbytné v běžném denním životě i upevnění pracovních návyků. Prostory denního stacionáře pro zajištění komplexní rehabilitace, prostory psychiatrické ambulance, ambulance klinické psychologie, pracovna sestry pro komunitní péči. Rekonstrukcí dále vznikne pracovna, zázemí pro sociálního pracovníka k zajištění návaznosti extramurální sociální péče a učebna k využití edukace pacientů psychiatrického oddělení následné péče i pacientů ambulantních a pacientů denního stacionáře. Současně uvolněním prostor prodejny potravin a bufetu v areálu ústavu dojde k snadnému vytvoření dostupného zázemí k nácviku sociálních aktivit – trénování nakupování, společného posezení v rámci principů kognitivně behaviorální psychoterapie, na které lze následně navazovat navyšováním nároků dané činnosti, a tak lépe připravovat pacienty na fungování v běžném domácím zázemí. Grant bude současně použit pro rozšíření spektra odborného vzdělávání zdravotnického personálu v oblasti sociální péče a podpůrných psychoterapeutických technik. Dále bude v rámci projektu pořízeno vybavení potřebné pro systém ucelené rehabilitace, zejména vnitřní vybavení vily, IT vybavení, počítačový program, cvičební pomůcky a stimulační hry. V rámci publicity projektu bude mimo jiné zajištěna mediální informovanost (zejména v lokálním a regionálním tisku) a proběhne slavnostní zahájení nového provozu, odborná konference pro odbornou veřejnost a den otevřených dveří pro širokou veřejnost. Dílčím cílem projektu je destigmatizace pacientů a psychiatrické péče jako celku.

AN OUTLINE OF THE DEVELOPMENT AND OF CURRENT STAGE OF THE ALBERTINUM ŽAMBERK PROJECT - September 2015

Let us now have a closer look at the project which is being currently accomplished under the name: “Albertinum Žamberk – complex rehabilitation in the psychiatric ward environment”. In the following text we will be dealing with the content, history and progress of the project.

We first obtained the information about the possibility of applying for the Norwegian funds grant in the programme CZ 11 – “Initiative in public health”  - AKTIVITA I. –psychiatric care, at the beginning of July last year from the head of Health Care Department of Pardubice Region Office Mr. Gregor.  It had been proven to us at that time that it always helps to be prepared because we had already done some preparations for the reconstruction of so called  “Doctors ´ villa” utilizing it for the needs of the Psychiatric ward. After evaluating our capacities we decided to hand in an application for the grant money. We also created an organisational and project team and despite an extraordinarily close deadline (the application was originally supposed to be handed by 29^th August 2014 later to be postponed till 12^th September 2014) we started our cooperation with Regional Development Agency of Pardubice Region (RRA Pk) on the administration of our application. Although it had seemed almost impossible in the beginning, we managed to hand in the application in time and so we could await the outcome of the assessment by the Ministry of Health – as a partner of the project and by the Ministry of Finance – as an intermediary of our project.

Among all the people involved we would like especially to thank Ms. Ing. Veronika Šimková from RRA Pk. She was responsible for the complete administration of the application.  Further acknowledgments should go to Ing. Tmejová and Ing. Věnečková from KIP Litomyšl Ltd. who should be thanked for creating the project documentation for the construction permit in shorter than usual time. And the members of the project team - Mgr. Petra Urbanová, Mgr. Marcela Tomanová, Bc. Hana Leichtová and Ing. Vladimír Fabián - deserve credit for administrating the co called “soft part” of the project and for commenting on the construction and equipment part of the project. Also the members of Pardubice Region council and assembly should be thanked as well as the Department of Health of the aforementioned institution for the support they gave to the project and for allotting us a subsidy of 4,25 mil CZK in their role of a co-funder of the project.

On the 15^th January 2015 we received the news about our application having been approved by the Ministry of Finance. We also had our preliminary estimation of overall expenses confirmed in the sum of 23 979 885, 00 CZK of which there is a part of Národní fond (National fund) subsidy of 19 183 907, 00 CZK which amounts to max.  of 80 per cent of the overall estimated expenses of the project. Our part of the investment therefore makes up the portion of 20 per cent i.e. 4 795 978, 00. This sum will be partly paid by the investment contribution of Pardubice region - 4 250 000, 00 - and partly paid by us. Our contribution will also include non-deductible costs and expenses which are not allowed to be paid by grant.

Immediately after we had received the news of our subsidy having been admitted to us we informed the office of Pardubice region and started with individual tenders for supplying the services needed for the project. We also set a time schedule of meetings of our project organisational team.  In five tenders that have been concluded so far an administrator of the project has been chosen as well as of two public procurements which became the Regional Development Agency of Pardubice Region (RRA Pk) and Mgr. Petra Srdínková was appointed the administrator of the project.  Also a project designing company has been selected - the KIP Litomyšl ltd. company which has prepared the project documentation for realization of the construction and will subsequently provide the service of authorized supervision over the construction itself. The tender for the service of the technical supervision over realization of the construction was won by the Hudeček ltd. company from Ústí nad Orlicí. A crucial moment was the choice of the actual construction company to realize the project. This tender was on the basis of a contract administrated by the Regional Development Agency of Pardubice Region - RRA Pk and the company chosen was Chládek a Tintěra, Pardubice a.s. . The last concluded tender by this day has been the choice of the coordinator of safety and health measures (BOZP). This obligatory function will be secured by a contract with Ing. Jan Dušek from Přelouč.

The first interim monitoring report, which covers the period between 14^th January 2015 and 30^th June 2015, including the application for the payment for invoicing the project documentation for realization of the construction, was handed to the Ministry of Finance for revision and control on 31^st July 2015. Now we are awaiting potential comments or suggestions for supplying further data.

Another essential step to be taken for securing continuous subsidizing of the project is the approval of the loan conditions by the council and subsequently by the assembly of the Pardubice Region. We provided for these negotiations the results of internal tenders and recommended a suitable provider of the loan. The Assembly of Pardubice region decided on the results on 17^th September 2015.

The main goal of the Albertinum Žamberk – complex rehabilitation in the psychiatric ward environment” project is to meet the demand for creating the necessary conditions for the development and implementation of a complex programme of medical and rehabilitation care in the environment of a psychiatric ward. This programme should enable better evaluation and observation of the patient´s condition and help to make the decision of placing the patient in some of the centres of community care outside the psychiatric hospital and, finally, should improve the gradual preparation of returning the patient in common life outside institutions. This should also help decrease the level of re-hospitalization of patients. The basis of the project lies in complexity which is understood as an integrated system of interventions in the area of physical health and mental health and also of social needs of the patient.  The concrete subject of our project is reconstruction of the building (so called “Doctors´ villa”) located within the premises of our institution because the aforementioned integrated system of care would be impossible to realize in the facilities which are currently used by our psychiatric ward. The reconstruction will create rooms for therapy and for group sessions as well as for rehabilitation procedures. There will also be a small kitchen and a day-common room for training communication, having meals together, training table manners and generally social interaction necessary for dealing with everyday life. There will also be a training laundry room, and other facilities focusing on training and fixing self-reliance and self-care in everyday life and also for improving some working habits. There will also be a day care centre for securing complex rehabilitation, also a psychiatric ambulance facility and clinical psychology centre and, also, a room for a nurse of community care.  The reconstruction will also create a room for a social worker who will provide the continuity of extra-mural social care and also a classroom for educational activities for the patients of the psychiatric ward, ambulant patients as well as day care centre´s patients. The project plans to use the facilities of today´s grocery shop and a buffet on the premises of our hospital and thus  a suitable environment will be created  for training social skills such as shopping, gathering and socializing  with other patients. In accordance with the principles of the cognitive-behavioural therapy the setting of various social situations can be changed and adjusted  and made more demanding and thus prepare the patients gradually for real life situations. The grant will also be used for extending the education and qualification of our staff in the area of social care and supportive psycho-therapeutical techniques. Another essential part of the project is the material equipment of the interior of the new facility in the villa i.e. IT hardware and software, physical exercise tools, stimulation games aids etc..  As a part of publicising of the project there will be an emphasis put on media coverage (with a focus on local and regional press). After the completion of the project an opening ceremony is intended to be held together with a conference for professionals and an “Open door day” where general public attendance will be welcomed. This corresponds with another major goal of our project – de-stigmatization of psychiatric patients and psychiatric medicine generally.
